Guest Agreement

This agreement is entered into by and between The Belmont Pet Resort and Daycare and Owner for the pets listed at the end of this document:

I agree to pay for all services due by the date of checkout or the pet(s) will not be released from The Belmont Pet Resort and Daycare. By signing this contract and leaving the pet with The Belmont Pet Resort and Daycare, I certify the accuracy of all information given about my pet(s). I specifically represent that I am the sole owner or agent of the pet, free of all liens and encumbrances.

I agree to these terms and conditions.

I understand that The Belmont Pet Resort and Daycare business hours are from 7:00am to 6:00pm Mon-Fri and 7:00am to 12:00pm Sat-Sun, unless otherwise posted. I understand the charging policy as follows: Daycare: check in time is between 7:00am-9:00am and check out time between 4:00 pm- 6:00 pm. If I pick my pet up after this check out time, I will be charged $10 past 6:15pm. If I do not pick up my pet by 6:30pm, my pet will stay the night and I will be charged the regular boarding rate. Late charges must be paid at the time of pick up. Boarding: check out time on any day is 7:00am to 12:00pm. If I pick my pet up after this check out time, I will be charged a full day of boarding for pick-ups between 12pm to close. Check in time is from 9am-3pm. Early and late check-ins are available for an extra charge of $10.00.

I understand that my dog(s) must be friendly towards other dogs and people. They must not be food or toy protective. Owners certify that their dog(s) have not harmed, caused death or shown aggression towards any person or dogs. I understand that if my dog has a history of aggression or biting, The Belmont Pet Resort and Daycare reserves the right to refuse service, and that all bites will be reported to the local authorities as required by law. I have disclosed to the Belmont Pet Resort and Daycare all known dangers associated with my dog.

I understand that if my pet becomes ill and requires professional medical attention, The Belmont Pet Resort and Daycare in its sole discretion may engage the services of a local veterinarian, any and all expenses will be paid by the pet owner. I authorize The Belmont Pet Resort and Daycare to [A] do anything needed for my pet’s health. This includes taking my pet to The Belmont Animal Hospital during regular business hours and to Nashville Pet Emergency Clinic after normal veterinary office hours. [B] do anything needed for my pet’s health but with a cost not to exceed $[C] please keep my pet as comfortable as possible, but do not do any extra veterinary care. (Please mark the appropriate box on sign in sheet). The Belmont Pet Resort and Daycare will always attempt first to call you and your emergency contact number left at drop off.

I understand that The Belmont Pet Resort and Daycare is a cage-free environment. I accept the risks involved and agree that the Belmont Pet Resort and Daycare is not liable for any injuries or illnesses resulting during my dog's visit. Even though The Belmont Pet Resort strives to provide the cleanest and safest environment for my pet, he/she may still be at risk of contracting communicable diseases such as the Canine Papilloma Virus (Puppy Warts), or the Bordetella virus, as well as injuries during playtime like broken nails, sore pads, puncture wounds, abrasions, and parasites.

I agree to release The Belmont Pet Resort and Daycare and its agents and employees from any and all liabilities or claim due to injury to myself, my pet, or any other property of mine, which may arise in any way out of services and/or products provided by or as a consequence of my association with The Belmont Pet Resort and Daycare.

I agree to these terms and conditions.

I release The Belmont Pet Resort and Daycare, its agents and its employees from any liability or claim due to the death of my pet, unless The Belmont Pet Resort and Daycare has been found negligent in the care of my pet. I understand that under no circumstances will The Belmont Pet Resort and Daycare liability exceed the lesser of the current chattel value of a pet of the same breed or species or the sum of $200.00 per animal.

To prevent the spread of diseases while my pet is at The Belmont Pet Resort and Daycare, all pets must be current on Rabies, DHLPP, Bordetella (even though most veterinary clinics require this vaccine annually, I understand the Belmont Pet Resort and Daycare requires it every six months), and a fecal exam every six months to check for intestinal parasites. Vaccinations must have been administered at least 72 hours prior to visiting. I understand and agree that it is my responsibility to provide proof of vaccination history for each animal that comes to The Belmont Pet Resort and Daycare premises. Although the Bordetella vaccine is available, there are some strains that it does not cover, and I understand that my pet is still at risk of contracting a virus at any time.

I specifically agree that The Belmont Pet Resort and Daycare shall in no way be responsible for death or injury should my pet contract canine cough or another illness while at The Belmont Pet Resort and Daycare. If my pet is found to have fleas and/or ticks, it will be treated by The Belmont Pet Resort and Daycare at my expense.

I agree to these terms and conditions.

In the case of minor stress related symptoms such as: stomach upset, vomiting, loss of appetite, minor scratches, I give The Belmont Pet Resort and Daycare permission to administer to my dog the following over the counter remedies if necessary: Endosorb (Doggie Pepto Bismol), Imodium, Benadryl, Triple antibiotic cream. If my pet requires any medication that is not in pill form, there will be a fee of $5.00 per application added to my bill. This includes eye, ear and topical medications.

I agree to these terms and conditions.

No dog may be abandoned at The Belmont Pet Resort and Daycare. I agree that I will not neglect to pick up my dog by the time he/she is scheduled to leave without providing notification. Any dog that is left at The Belmont Daycare without any contact, instruction or notification from me, or without indication of the ability, willingness or plans to pick him/her up (by myself or my personal representative or agent) will be considered abandoned upon the seventh day of such notification failure. I understand that if I abandon my dog at The Belmont Daycare, The Belmont Pet Resort and Daycare will, by default, become the legal owner and guardian of the dog.

I understand that by allowing my pet to participate in services offered by The Belmont Pet Resort, I hereby agree to allow The Belmont Pet Resort to take photographs or use images of my pet(s) in print form or otherwise for publication and/or promotion.
